Personal Data: Any information regarding an identified or identifiable natural person.

Our health institution, which is the data controller in accordance with Article 3/f of the Personal Data Protection Law No. 6698 (KVKK):

Purpose of Processing and Transfer of Personal Data

For these purposes, personal data must be processed in line with our current duties and responsibilities.
Processing of Personal Health Data
Regulation on Processing and Ensuring the Privacy of Personal Health Data (Official Gazette: 20.10.2016 – 29863, Article 7):
Personal health data; It may be processed without the express consent of the person concerned by persons under the obligation of confidentiality or authorized institutions and organizations for the purpose of protecting public health, preventive medicine, medical diagnosis, execution of treatment and care services, planning and management of health services and their financing.

Protection of Your Personal Data
All your personal data recorded by us electronically and physically are protected in accordance with Article 3 and Article 4 of the Decision No. 2018/10 of the Personal Data Protection Board dated 31/01/2018 regarding the Adequate Precautions to be Taken by Data Controllers in the Processing of Special Personal Data. .

(As per KVKK Articles 8/b and 9/2): Provided that adequate precautions are taken, your personal data may be transferred if one of the conditions specified in the third paragraph of Article 6 is met.
Your Rights Regarding Your Personal Data
In accordance with KVKK Article 11:
- Learning whether personal data is processed or not
- Requesting information if personal data has been processed
- Learning the purpose of processing personal data and whether they are used for their intended purpose
- Knowing third parties to whom personal data is transferred domestically or abroad
- Requesting correction of personal data if it is incomplete or incorrectly processed
- Request deletion or destruction of personal data (excluding legal retention periods)
- Objecting to the emergence of a result against the person by analyzing the processed data exclusively through automatic systems
- Requesting compensation for damage in case of damage due to illegal processing of personal data
Ways to Apply to the Data Controller
KVKK Article 13:
- You can submit your requests to the data controller in writing or by other methods determined by the Board.
- The data controller will finalize your request free of charge as soon as possible and within thirty days at the latest.
- If the request requires an additional cost, the fee in the tariff determined by the Board may be charged.
- If your request is accepted, it will be fulfilled; if it is rejected, the reason will be explained and you will be notified.
Complaint to the Board
KVKK Article 14:
- In cases where the application is rejected, the response is found insufficient, or the response is not given in time, you may file a complaint with the Board within thirty days from the date you learned the response of the data controller, and in any case within sixty days from the date of application.
Procedures and Principles of Investigation upon Complaint or Ex officio
KVKK Article 15:
- The Board conducts an ex officio investigation upon complaint or if it learns of an alleged violation.
- The Board examines the request and responds to those concerned. If no response is given within sixty days from the date of complaint, the request is deemed rejected.
- If a violation is detected, the Board decides that the unlawfulness will be remedied by the data controller and notifies the relevant parties. This decision shall be implemented without delay, within thirty days at the latest.
- If it is determined that the violation is widespread, the Board takes a principle decision and publishes it.
- In case of irreparable or impossible damages and if there is a clear violation of the law, the Board may decide to stop data processing or transfer of data abroad.
