It may be processed without the express consent of the person concerned by persons under the obligation of confidentiality or authorized institutions and organizations for the purpose of protecting public health, preventive medicine, medical diagnosis, execution of treatment and care services, planning and management of health services and their financing.<\/p><h3>Personal Data That Can Be Recorded During Institution Visits<\/h3><p><strong>Private Hospitals Regulation (Official Gazette Date: 27.03.2002, Number: 24708, Article 48):<\/strong><\/p><ul><li>Patient admission sheet (Name, surname, TR ID number, passport number, place of birth, year of birth, marital status, gender, address, telephone number, etc.)<\/li><li>Medical observation and examination paper<\/li><li>sick sign<\/li><li>Nurse observation sheet<\/li><li>X-ray and laboratory request sheets and examination reports<\/li><li>surgical paper<\/li><li>Patient&#039;s examination request form<\/li><li>Release summary<\/li><li>Treatment acceptance consent form<\/li><li>Appointment information<\/li><li>E-mail address and other contact data<\/li><li>Personal data obtained through voice conversation recordings and e-mail, letter or other means<\/li><li>Bank account number, IBAN number, credit card information, billing information<\/li><li>Data on private health insurance<\/li><li>Social Security Institution data<\/li><li>Camera recordings for security and inspection purposes<\/li><li>Vehicle license plate information in case of parking lot use<\/li><li>Job application forms<\/li><li>Employee personnel files<\/li><\/ul><h3>Protection of Your Personal Data<\/h3><p>All your personal data recorded by us electronically and physically are protected in accordance with Article 3 and Article 4 of the Decision No. 2018\/10 of the Personal Data Protection Board dated 31\/01\/2018 regarding the Adequate Precautions to be Taken by Data Controllers in the Processing of Special Personal Data. .<\/p><h3>Institutions to which your personal data may be transferred<\/h3><ul><li>Ministry of Health<\/li><li>SGK<\/li><li>Contracted laboratory companies<\/li><li>private insurance companies<\/li><li>Contracted archive companies<\/li><li>Hospital information management system providers<\/li><\/ul><p><strong>(As per KVKK Articles 8\/b and 9\/2):<\/strong> Provided that adequate precautions are taken, your personal data may be transferred if one of the conditions specified in the third paragraph of Article 6 is met.<\/p><h3>Your Rights Regarding Your Personal Data<\/h3><p><strong>In accordance with KVKK Article 11:<\/strong><\/p><ul><li>Learning whether personal data is processed or not<\/li><li>Requesting information if personal data has been processed<\/li><li>Learning the purpose of processing personal data and whether they are used for their intended purpose<\/li><li>Knowing third parties to whom personal data is transferred domestically or abroad<\/li><li>Requesting correction of personal data if it is incomplete or incorrectly processed<\/li><li>Request deletion or destruction of personal data (excluding legal retention periods)<\/li><li>Objecting to the emergence of a result against the person by analyzing the processed data exclusively through automatic systems<\/li><li>Requesting compensation for damage in case of damage due to illegal processing of personal data<\/li><\/ul><h3>Ways to Apply to the Data Controller<\/h3><p><strong>KVKK Article 13:<\/strong><\/p><ul><li>You can submit your requests to the data controller in writing or by other methods determined by the Board.<\/li><li>The data controller will finalize your request free of charge as soon as possible and within thirty days at the latest.<\/li><li>If the request requires an additional cost, the fee in the tariff determined by the Board may be charged.<\/li><li>If your request is accepted, it will be fulfilled; if it is rejected, the reason will be explained and you will be notified.<\/li><\/ul><h3>Complaint to the Board<\/h3><p><strong>KVKK Article 14:<\/strong><\/p><ul><li>In cases where the application is rejected, the response is found insufficient, or the response is not given in time, you may file a complaint with the Board within thirty days from the date you learned the response of the data controller, and in any case within sixty days from the date of application.<\/li><\/ul><h3>Procedures and Principles of Investigation upon Complaint or Ex officio<\/h3><p><strong>KVKK Article 15:<\/strong><\/p><ul><li>The Board conducts an ex officio investigation upon complaint or if it learns of an alleged violation.<\/li><li>The Board examines the request and responds to those concerned. If no response is given within sixty days from the date of complaint, the request is deemed rejected.<\/li><li>If a violation is detected, the Board decides that the unlawfulness will be remedied by the data controller and notifies the relevant parties.
